How Al Quddus Medical Centre collects, uses and protects the information you give us, in line with Uganda's Data Protection and Privacy Act, 2019.
When you book an appointment we ask for your name, telephone number, optional email address, the department you need and the date you would like to come. At the front desk, if you register as a patient, we also record your date of birth, address, next of kin and the clinical notes made during your visits.
To contact you about your appointment, to keep an accurate record of the care we have given you, and to meet the record-keeping duties placed on medical practitioners.
Clinical notes are visible only to doctors and nurses at the centre. Front-desk staff can see your contact details and appointment history but not your clinical notes. Every time a member of staff opens a patient file, the system records who opened it and when.
Appointment enquiries that do not become visits are deleted after twelve months. Patient records are retained for as long as required by medical record-keeping rules.
You may ask to see the information we hold about you, ask us to correct anything that is wrong, or withdraw your consent for us to contact you. Write to info@alquddus.co.ug or speak to the front desk.
This is a starting point written for the website. Have it reviewed by a lawyer before the centre relies on it, and register with the Personal Data Protection Office as a data collector.
